Purpose of the Report
- This report provides information on Family Medical Leave (FMLA ) request submitted by employee(s) in specified home organization.
Report Usage
- This report is useful when tracking and monitoring FMLA time for an employee.

Report Parameter Definitions and Usage
- Year: Year refers to the calendar year in which the FMLA request was submitted.
- Note: Because the report is based on the calendar year in which the FMLA request was submitted, a timing issue could result at the end or beginning of a calendar year.
- Division: Two-digit division code.
- Home Orgn: After selecting the division code, the screen will self-populate with a list of applicable home organization codes. You have the option to select one, all or a variety of home organization. This selection can be made by holding down the Ctrl key or you may selection all or deselect all home organizations as shown below. You should select finish to run this report.
Report Output
- The report provides information on employees who applied for FMLA during the calendar year within a division and is sorted by employee’s home organization. This report will list each employee’s banner id, name (first/last), position title, e-class, reason code for FMLA, status – approval/denial, begin and end dates of the leave, days reported in Banner HR as FMLA, hours used and allowed as of the last pay period, and hours left for FMLA (see example below).
Banner Data Verification
- Information recorded as FMLA can be verified using PEIATND (Employee Attendance Inquiry). As you can see, Mr. Tull has used a total of 22.50 hours for FMLA as verified on PEIATND based on calendar year.

| Position Title | Employee_Position description. Defaults from the position title if left blank. |
| Employee E-Class | Employee class code. Organizes position information consistent with employee groupings. At the employee level,used to help determine controls for benefits, leave, payroll and self-service rules. |
| Reason Code | A code identifying the reason of the leave of absence for the employee. |

| Leave End Date | Anticipated end date for an employees leave of absence. |
